Springfield City School District is excited to announce that FinalForms for the 22/23 school year is open and available for you to complete your student(s) back to school forms. These forms are required to be completed before the first day of school.
If you have any of following situations please upload appropriate documentation:
Address change, please upload a proof of residency document (lease/mortgage agreement or utility bill within the last 60 days - name, date, and address MUST be displayed).
Any change in custody will also need a court stamped custody document uploaded.
If you are in the following grades: Kindergarten, 7th or 12th, updated immunizations are required. Please upload updated immunization records. An immunization schedule has been attached to this email. Please make an appointment with your family doctor, The Rocking Horse or contact the Clark County Health Department.
